If you have completed a Supplier Profile Form before, some of the information previously provided may be included on the attached form. ​ Please update any incorrect information provided and complete the rest of the form.

If you are a certified Minority (MBE) or Women-Owned Business Enterprise (WBE) in your state, or if you are a Certified Disadvantaged Business Enterprise (DBE) under the federal program, please provide a copy of your certificate or letter of self-certification when completing the Supplier Profile Form. ​ A postage-paid return envelope is also enclosed for mailing National Fuel the completed form and any necessary attachments.
